Failure Analysis
Failure Analysis of fixed equipment typically involves determination of the stress state and damage mechanisms that contributed to the failure. In many cases, there is indecision over the exact root cause due to an inability to adequately rank the factors. By reconstructing the failure using advanced analysis techniques, it is possible to rank causal factors and determine both the root cause and significant secondary contributing factors. In addition, this ranking enables any proposed solution to be focused on the appropriate factors
At IES we conduct assessments that determine the root cause through examination of the evidence and/or reproduction of the failure mode using advanced analysis techniques. We also have an in-depth understanding of equipment design, analysis and repair, so are able to offer not only a determination of root-cause, but also practical guidance on solutions in order to eliminate further failures.
